Dear All

Requisitions are being submitted for signature without all of the necessary information to facilitate their complete processing.

In an effort to eliminate or at least minimize unwanted delays, we are asking that you ensure the inclusion of the following information on all future requisitions:

1. A clear, concise description of the requested items or items.

2. A clear justification of requested items.

3. An indication of prior budget approval for all requested items costing in excess of US $500.00.

4. A budget change form along with the requisition form for items that are costing in excess of US $500.00, that do not have prior approval.
