Forms to Be Returned
The following forms must be downloaded, printed, filled out, and returned to your Admission Counselor by July 15, 2008. A pre-printed envelope has been included in the preregistration package. The University requires the listed information to help you become better acclimated to your new location upon your arrival in Grenada.

If you have not already submitted your Health Form, please do so. Return them in a separate envelope to Ms. Susan Conway, RN, Dir., Student Health Records Dept., c/o University Support Services, LLC, One East Main Street, Bay Shore, New York 11706-8399. This form must be returned in order for you to receive your registration voucher letter.
We urge you to complete your Health Forms before you arrive. This is important to both your health and the health of others in the SGU community. Completing your Health History Form before you arrive will speed up your registration process will provide for a smooth transition.
Health History Form:
All students are required to sign a copy of the “Medical School Consent for USMLE Transcript Release” form. This form must be completed for each student before he/she can be certified to sit for the United States Medical Licensing Examination.
